SMS Opt-In Example / Consent Flow

Consent is collected through explicit user action only — the checkbox is unchecked by default and is never pre-selected or implied.
1. Example Consent Checkbox
The following is the exact checkbox and disclosure text shown to participants before they submit consent. The checkbox is unchecked by default. The participant must manually select it to opt in to SMS.
Note: The checkbox above is shown in its default unchecked state as it appears to the participant. It is rendered here as disabled for illustration only.
2. Checkbox Behavior
The SMS opt-in checkbox is unchecked by default and must be manually selected by the recipient before consent is recorded. SMS consent is optional and separate from account registration and acceptance of the Terms of Use or Privacy Policy. It is not required to receive services or to schedule an appointment. If the recipient does not opt in to SMS, they may use non-SMS communication methods (such as email) where available.
SlotWise Pro does not treat purchased, rented, or scraped phone lists as valid opt-in. Consent must be given directly by the individual whose phone number is provided.
3. Consent Records
When a participant selects the SMS opt-in checkbox, SlotWise Pro records the following consent metadata:
- Consent timestamp (date and time of opt-in)
- Source page (the URL where consent was collected)
- Phone number provided by the participant
- Exact consent language shown to the participant at time of opt-in
These records are retained in accordance with applicable regulations and are available for compliance review upon request.
4. Message Frequency and Rates
Messages are transactional and service-related. Message frequency varies based on appointment or event activity and the participant's communication preferences. Msg & Data rates may apply.Wireless carrier charges may apply based on the participant's mobile plan. Mobile carriers are not liable for delayed or undelivered messages.
5. Opt-Out (STOP / HELP)
Reply STOP to unsubscribe from SMS messages at any time. Reply HELP for help or support information. After sending STOP, the participant may receive one final confirmation message confirming they have been unsubscribed. No further messages will be sent after opt-out.
