SMS Consent / Proof of Opt-In

Consent is collected only through a documented, user-initiated workflow and is not pre-selected.
1. Consent to Receive SMS
By providing your mobile number and affirmatively opting in through a SlotWise Pro web form, scheduling workflow, or other consent capture flow, you consent to receive transactional, service-related SMS messages related to scheduling operations, including confirmations, reminders, rescheduling notices, and appointment/event updates from SlotWise Pro on behalf of the organization using the service.
2. Opt-In Methods
Consent is collected only through user-initiated, explicit opt-in actions (for example, checking an unchecked consent box, submitting an intake/scheduling form that contains SMS consent language, or providing consent through a documented workflow). On the SlotWise Pro booking flow, the SMS checkbox is unchecked by default and must be manually selected by the recipient. SMS consent is optional and is not required to receive services or schedule an appointment. If the recipient does not opt in to SMS, they may use non-SMS communication methods where available. SlotWise Pro does not treat purchased, rented, or scraped phone lists as valid opt-in.
For compliance records, SlotWise Pro stores consent metadata, including consent timestamp, source page, phone number, and the exact consent language shown to the recipient.
3. Message Type and Frequency
Messages are exclusively transactional and service-related: appointment confirmations, reminders, rescheduling notices, cancellation notices, and related appointment status updates. SlotWise Pro does not send marketing, promotional, affiliate, or lead-generation SMS under this program. Message frequency varies based on your appointment or event activity, typically 1–5 messages per appointment or event.
4. Fees and Carrier Disclaimer
Msg & data rates may apply. Wireless carrier charges may apply based on your mobile plan. Mobile carriers are not liable for delayed or undelivered messages.
5. STOP / HELP
Reply STOP to unsubscribe at any time. Reply HELP for help. After you send STOP, you may receive one final confirmation message confirming you have been unsubscribed.
6. Consent Is Not a Condition of Purchase
Consent to receive SMS messages is not required as a condition of purchasing goods or services. Users can use alternative contact methods where offered by the organization.
7. Eligibility and Number Ownership
By opting in, you represent that you are the account holder or authorized user of the mobile number provided and are permitted to receive messages at that number.

9. Customer Compliance Responsibility
Organizations using SlotWise Pro are responsible for ensuring their consent collection, disclosures, and messaging practices comply with applicable laws and industry rules (including TCPA, state privacy laws, and CTIA messaging principles).
10. Step-by-Step Message Flow (A2P Opt-In Process)
The following describes the exact opt-in process used by SlotWise Pro. This narrative is provided for carrier, Twilio, and regulatory review.
- The end user (participant, patient, student, or attendee) navigates to the SlotWise Pro booking or enrollment workflow provided by the organization.
- The user enters their mobile phone number in the designated field.
- The user is shown an SMS consent checkbox. The checkbox is unchecked by default and must be manually selected by the user to opt in.
- The checkbox disclosure text reads: “I agree to receive transactional SMS appointment notifications from SlotWise Pro, including appointment confirmations, reminders, rescheduling notices, cancellation notices, and related appointment status updates. Message frequency varies, typically 1–5 messages per appointment or event. Msg & Data rates may apply. Reply HELP for help or STOP to opt out. Consent is optional and is not required to receive services or schedule an appointment. - SMS consent is separate from acceptance of the Terms of Use or creation of a SlotWise Pro account. Users may proceed through the workflow without selecting the SMS checkbox.
- If the user does not opt in to SMS, they may receive appointment communications through alternative channels (such as email) where offered by the organization.
- When the user manually selects the checkbox and submits the form, SlotWise Pro records: consent timestamp, source page URL, phone number, and the exact consent language displayed at opt-in.
- Following confirmed opt-in, SlotWise Pro sends only transactional appointment SMS messages: confirmations, reminders, rescheduling notices, cancellations, and related service updates.
- The user may opt out at any time by replying STOP. Reply HELP for support information.
